Parental Agreement to Administer Prescription or Non-prescription Medicine

From time to time children will require prescribed medicines to be administered in school. Attached is a form to complete which will enable staff members to do so on behalf of the parent/carer.  Without this completed form we are unable to give a child medication.  When necessary and wherever possible to reduce paperwork into school, please complete and email the form to the school office on  Medicines should then be handed to the staff member on entry to school for safe keeping.
